Please make sure that you allow a minimum of two weeks prior to the due date of your FMLA, when you submit it to us for completion. Also please make sure your portion is completed and signed before submission.

- Returned checks will be charged a fee, plus bank fees and the amount of the check, and future payments by check will not be accepted.
- A fee will be assessed for forms requiring physician’s documentation. This includes but is not limited to FMLA and disability. A fee is not assessed for letters including “sick notes” issued during an appointment.
- A fee will be assessed for “injection only” appointments that do not require seeing a physician. This includes injections such as testosterone and B12.
- We offer Injection training on site for all injections for a fee
- A missed (no show) appointment will be charged a fee. Same day cancellations are considered a missed appointment. please call ahead for scheduling availability. Cash pay patients are always welcome . Clients must be 18 years of age or older to be seen.
Controlled Substances
Pursuant to Florida State Legislative Bill CS/CS/HB 7095 (http://www.myfloridahouse.gov/sections/Bills/billsdetail.aspx?BillId=46705) (effective July 1, 2011), your physician can no longer write prescriptions for certain controlled substances. If you need pain medications, you will be referred to Pain Management. These rules apply to NEW and EXISTING patients. If you have any questions about these rules, please speak with the office or your doctor. All controlled substance prescriptions and refills require an office visit.
